This page is where you’ll find all the forms you’ll need to register for lessons at PIOM. The Registration Form and School Policy will need to be printed, signed and returned for all new students.

PIOM School Policy Form

The School Policy Form includes such things as Tuition and Payment Options, Makeup Policies, Recitals, Parent Responsibilities etc. This form must also be signed and returned with the Registration form.
